When I try to visit the site I am receiving the following errors. Any suggestions

Yes, either get your host to set your PHP version back to a version suited for phpBB 3.0.12 (or copy you board to a PWS with a supported PHP version) then clear everything superfluous on your board.
Then use the STK to make sure your board is ready for upgrading.

If you have mods, the data of which can be used with phpBB 3.3.11 extensions (Gallery and ABBC) read the extension topics on how to upgrade.
Then, either

on PWS: switch to a PHP version compatible for 3.3.11 and upgrade, and then transfer the board back to the host (with a PHP version for 3.3.11)

or

switch host PHP version, import the sanitised database and upgrade.

Your custom header can probably still be used
